That happens. I've seen it in Linckia sp. too. Nothing "missing" just came to being with 4 (or even 6) legs.
Sand stars are pretty good at regenerating lost limbs, fromias are too. Linckias don't seem to fare that well with regenration, at least in captivity from what I've seen but more or less "legs" happens quite frequently.
