Question on CO2 regulator

I know there're a number of divers here, hoping someone can clarify some regulator questions;

In the last month or so, the connection where the regulator screws onto the CO2 tank has started loosening itself. It starts out wrench tightened, then somehow becomes just finger tight, losing all the CO2.

This hasn't always happened, just started doing it recently. It has new washers on it.

My husband thinks it's happening when the CO2 tank warms up, but that doesn't explain why it's only started happening lately (since the CO2 tank has always changed temp, but the loosening is only a recent issue).

As pic shows, it's got the two gauges, the first gauge shows tank pressure, and the second gauge shows pressure after the pressure reduction valve. Then there's a needle valve and solenoid. It's the standard regulator sold at Marine Depot.

My husband believes that the open/shut valve at the top of the CO2 tank (not shown in pic) is an 'either opened or closed' deal - rather than something that needs to be set at a particular reading. The regulator instructions say to open that tank valve until the 1st gauge reads about 800 psi, but wouldn't this reading change all the time depending on temperature of the CO2 tank and how full the tank is? In that case, it's shouldn't make any difference, right?

Could the 1st gauge setting possibly be what's causing the regulator to loosen itself?

Anyone have any other ideas why this could be happening? TIA

Thanks, xtopher,
Nope, there's a black fiber washer that gets replaced every tank fill. It's been replaced a half dozen times since this started happening.
