ID this swimming worm..

This little guy comes out every night after the actinic goes out.. I can watch him swim under the LED lights. He's a fast little bugger - swims all over the tank. I finally got an opportunity to photograph him when he rested on the glass for a few minutes.
Any ideas on what it is? Should I remove him/her? I think I'll name it "Pat".
