If it actually does improve appetite, then garlic probably works on several levels:
Most fish carry intestinal parasites. Simply clearing these away, a garlic has been amply shown to be capable of, improves gastrointestinal health (and arguably clears space in the gut for more food). Garlic may also work against any minor bacterial diseases the fish may be suffering (the pathway may be vague at this point, but garlic contains several sulfurous components that are effective antimicrobials)... which could then lead to improved appetite.
