okay fish dying

i have a 12 gallon nano. it has been going for 6 months now. i started with a few fish and stoped at four, a clown fish a royal gramma a blue regal tang and a 6 line wrasse. they were fine for a while, i started adding coral, LPS, mushrooms, polyps etc.so i rearenged the rock about a week ago so i could save some room. i took all the coral out and left the fish in the water thinking they would move and i rearraged the rock. put al the coral back in and kept everything going. about 2 days after that 2 of my fish acted weird.my royal gramma started staying in the same spot alot of the time and my blue tang started looking like it was being beatin up by something.2 days ago my blue tang was like just on the sand dead looking (still breathing though) so i picked him up with my fingers and he was still breathing i put him in my dads tank in hope of saving him. he died. and today i found the royal gramma dead in the filter. But all my corals are perfect (except xenia, but that was from to much current) and the clown fish and 6 line wrasse are good to. i pulled the gramma and tang out and they had kind of a silverish thing on there skin, either it was a deasease or just how fish look out of water, i dont know
but can anyone tell me if this is my falt, bad water or somthing, or just a freak thing

tanks specs
temp =77-79
nitrates =15-55 ppm (it varies)
ammonia and nitrites= 0
calcium = 380
ph = 8.0- 8.2
pleaseeee help

that is way to many fish in that size tank. it could be stress from the small tank, even the smallest tangs need a 4' tank. the larger tangs need at least a 6' tank. i put the blue tang in the 6' tank personally.

did the fish have ich? how dirty was the water during the rearrangements?

Agree the tank was over stocked. Big thing is the rearranging of rock. Sand gets stirred up and toxins are released. When I reaquascape I try not to move the lower rocks so I don't disturb the sandbed. If I do have to disturb the sand I have nsw ready for a waterchange and do some heavy siphoning. BTW Welcome to TRT. This is a great place to learn and just have fun.
