Banggai Cardinal Death?

I placed 5 Banggai Cardinal in my tank a week ago I acclimated them for 2 hours before adding them, the LFS had them for over 3 weeks. One by one they seem to get sucked or stuck on my Koralia 4 powerhead on the top right corner near my overflow. Each morning I look at the tank one is stuck (dead) I have two more Koralia 4 in the tank in which they have not gotten stuck too. I have one more cardinal left swiming ......... They were eating frozen shrimp as well..... What gives? any ideas?

Couple things,How big is your tank(hope pretty big,because adding five is alot of load to a new or small tank).What are all your water parameters?

90 gal... tank been running for a few months with live rock live sand added some crabs and snails once cycle was completed.... water para I will have to get back on... when first one died I did a 20% water change 3 days ago.... (about 20 gallons)

That sounds good.Did you accumate them and how?And I 'll wate till you do all your checks.

I acclimated them for 2 hours in the styrofoam container I picked em up in using an air line from my tank slow dripping water into it, increasing flow every 20 mins...I ran out of testing supplies.. gotta visit LFS


And I did also add a large feather duster into the tank as well.... my GF loves them

Well last one went today....argh.... frustrating I caught this one on the bottom of the tank half dead.... a snail went to get it and he / she swam away sinking to bottom inch away again.... nothing else in the tank seems to be doing bad... ( 1 large feather duster, One green shroom hitch hiker from a live rock, yellow poloyps frag, green zoa frag some snails and red leg crabs..)

any other test I should look into?

also when I scooped him / her out I seen what looked like a small white worm in the net as well.. small I mean like a 12 font size tin as a pin...lights were off and I used a flash light to examine the fish in the net against the glass before removing.. this is when I saw it....all that was on was my moon lights.. this thing wingled away
