I like it. Might be better to not call it an "application" though since everyone that wants to join the club can without needing to worry about being turned away. You should also make sure that you state what information is required to become a club member and that the rest of the information is just to help the club members to get to know one another.

Do you guys have a members only section for people once they have enrolled? What is the benefit to filling this out? Just a thought on some info to post.

I am starting a database on the members. I don't know about a members only section since we are still small... maybe in the future.

Personally I don't like the members only section. Say if someone is interested in the club and they want to see something its discourageing.
