A sterilizer is just a tool, like all the others. The problem with ick is that they have an "egg" phase and a free swimming stage. A UV can help keep an ick outbreak under control by killing the free swimming stage, but can't do anything about the stage that isn't in the water column. It also can't help fish already infected.
Think of a sterilyzer like washing your hands. It helps reduce the chance of disease, but can't stop it altogether. Just provides a safer environment. If fish can stay healthier this way, then they have even more of a chance fighting off disease on their own.
